Release Form and Policies
The signed participant has my permission to participate in the Midwest Athletics (MWAA) Honor Roll Football Showcase. I am aware that the participant’s contact information will be shared with college football coaches for recruiting purposes only. No other entity shall receive the participants contact information. I am giving permission to use any photos taken at the event of the signed participant on our MWA website.
I understand and accept the condition that neither the MWA, it’s directors nor coaches or the site Hillsdale College will assume responsibility for medical and dental expenses incurred as a result of participation in this camp. I confirm that the participant has personal medical insurance coverage and that any expenses incurred while at the camp is my responsibility. In case of an emergency, I understand that every attempt will be made to contact the person listed. If contact is unsuccessful, I give permission to the attending medical personnel to render medical treatment to the participant.
Refund Policy:
Cancellations must be done by email only by May 23rd, 2020. If a cancellation occurs after May 23rd, 2020 no refund will be made. This includes all injuries from other sports or activities. If you fail to show up and you call us after the event no refund will be given back. Refunds are given back if you provide notice prior to May 22nd, 2018. We will charge a $15 processing fee. A check for the difference will be mailed back to you.
Cancellations before May 23rd, 2020, Refund minus $5 processing fee.
Cancellations on/after May 23rd, 2020 no refund.
All Receipts are sent via Email.
